Insurance or Other Payor Coverage
Patients With Insurance Coverage:
Please understand that insurance is a contract between you and your insurance company. It is always recommended that you review your policy’s coverage for the planned procedures prior to scheduling with a medical and/or dental specialist in order to determine your in-network and/or out-of-network benefits. The Doctors of Oral Surgery Associates are in-network for many medical and dental plans but not all of them.
Every effort will be made to closely estimate your co-payments and deductibles, which are due at the time of service. With all due respect, we submit this reminder: you are responsible for all fees charge by this office, regardless of your insurance coverage.
Please reference our “Financial Policy” for additional information concerning payment.
Medicare Patients:
Oral Surgery Associates has elected to “Opt-Out” of Medicare and therefore we are not allowed to submit a claim to Medicare for any service furnished to a Medicare beneficiary.
If you have any additional questions concerning Medicare coverage for dental services, you can contact Medicare at 1-800-633-4227.
Personal Injury Cases:
This office does not accept liens, for auto-accident, other liability, or lawsuit-related cases. The patient is responsible for services provided at the time of service. Oral Surgery Associates will only file accident-related injury claims to the patient’s medical insurance. All other arrangements for injury reimbursement are the patient’s responsibility.
Workers Compensation:
If your injury is work-related, we will unable able to accept you as a patient.
