Insurance or Other Payor Coverage

Patients With Insurance Coverage:

Please understand that insurance is a contract between you and your insurance company. It is always recommended that you review your policy’s coverage for the planned procedures prior to scheduling with a medical and/or dental specialist in order to determine your in-network and/or out-of-network benefits. The Doctors of Oral Surgery Associates are in-network for many medical and dental plans but not all of them.

Every effort will be made to closely estimate your co-payments and deductibles, which are due at the time of service. With all due respect, we submit this reminder: you are responsible for all fees charge by this office, regardless of your insurance coverage.

Please reference our “Financial Policy” for additional information concerning payment.

Medicare Patients:

Oral Surgery Associates has elected to “Opt-Out” of Medicare and therefore we are not allowed to submit a claim to Medicare for any service furnished to a Medicare beneficiary.

If you have any additional questions concerning Medicare coverage for dental services, you can contact Medicare at 1-800-633-4227.

Active Military/Coast Guard:

Please make sure that you have an Appointment Control Number assigned by United Concordia-ADDP prior to you office visit.

Or; if you are considered remote you will need a “Command Memorandum” from you base Commander.

Personal Injury Cases:

This office does not accept liens, for auto-accident, other liability, or lawsuit-related cases. The patient is responsible for services provided at the time of service. Oral Surgery Associates will only file accident related injury claims to the patient’s medical insurance. All other arrangements for injury reimbursement are the patient’s responsibility.

Workers Compensation:

If your injury is work-related we will unable able to accept you as a patient.
